At Connected2Support C2SWA LLC, we monitor the physical, mental, and emotional well-being of our residents. We observe and check their progress in terms of health and quality of life. For the entire duration of their residence in our adult family home, we will ensure their overall wellness through systematic reviewing and reporting of any significant health changes to their primary care providers. We also assist with medication administration and ensure their physicians’ instructions are strictly followed. We are also staffed with an on-call nurse and dietitian to assist our residence with their care needs.
